Transitions: Wandsworth Mind Service For Care Leavers

Are you a young person who is leaving care who has mental health needs or do you know someone who could benefit from additional support. Read more about the service available in Wandsworth.

What the service can give you

Wandsworth Mind is keen to spread awareness of its Transitions service, which supports young people coming out of care who are experiencing mental health distress.

The service is aimed at young people with mild to moderate mental or emotional health needs, or are vulnerable to this.

Please note: this service is not intended for those with severe forms of mental health distress.

Staff aim to empower young people and have strong awareness of the difficulties and challenges that young people in care and care leavers face, including the impact on mental health.

They can help you:

  • Gain emotional resilience
  • Give you the skills to manage independent living
  • Access creative workshops, Talking workshops and 1-1 support in a therapeutic environment

Develop your:

  • Insight
  • Confidence
  • Self-esteem
  • Resilience & self -management skills

They will also encourage you to:

  • Develop your interests
  • Connect with Peers
  • Become more active in the community

Transitions accepts self-referrals and referrals from relevant professionals.

The service is held at Wandsworth Tooting Wellbeing Hub, 201-203, Tooting High Street, SW17 0SZ fortnightly on Tuesday evenings from 6pm to 8.30pm.
