Connecting your Care Launches in SW London
The South West London Health and Care Partnership (SWL HCP) provides health and care in the six south west London boroughs (Croydon, Sutton, Kingston & Richmond, and Merton & Wandsworth).
In order to make it easier for health and care professionals to access an individual's medical records when they are held by another health and care organisation, they are launching something called Connecting your Care. Connecting your Care will mean that certain people, including your GP, hospital doctors and nurses will be able to immediately see important information about you through a secure system. This could help them make the best decisions about your care and could be lifesaving in an emergency situation.
The system will not collect or record any new information about you; it will only show important information about you held in several different places in a “read-only” format. This means that the information cannot be changed, only read.
Systems similar to Connecting your Care already exist in other parts of London and SWL HCP is working closely with some of the technical experts who worked on those systems to build on, and benefit from, their knowledge and experience.
