Connecting your Care launches in SW London

A new system called Connecting your Care is being introduced in South West London that aims to improve the way health and social care professionals are able to access important information about you to help them treat you more quickly.

How will this benefit you?

Connecting your Care will mean that health professionals, including your GP, hospital doctors and nurses will immediately be able to see important information about you through a secure system. Access to that information could help them make the best decisions about your care and could be lifesaving in an emergency situation.

What information will be collected?

The system will not collect or record any new information about you; it will only show important information about you held in several different places and in a “read-only” format. This means that the information cannot be changed.

It also means that you only have to share your history once, and then other halth and care professionals can simply confirm it with you.
